0796dfff89 use FLASH_BANK_COMMAND_HANDLER macro 
Defines all flash_bank_command handlers using the new macro.

041c319d9d flash/<various>.c: remove useless declarations 
Remove useless forward declarations.
Moves flash structure definitions to end of files.
